When you have set up your Smart Form(s), you also want to make sure they are displayed somewhere so that players find them, and can smart submitting their information to them.
In order to determine where on your club page you want them to be displayed, go to:

2️⃣ The Club
3️⃣ Customization
Under Club Home and Club Information, you have the ability to add the Smart Form section and also decide where on each page you want to place it.
You can also scroll down to Custom Titles below the section that you are currently on, and name your Smart Forms section in mobile app with any title you want, such as Forms, Applications, Submissions, or whatever suits your club the most.
