How do I show Smart Forms on my club page?

Add the Smart Forms module to your club home or profile page in the app, and give it your own title.

Add the Smart forms module to your club's page in Settings → The Club → Appearance. You can place it on Club Home or Club Profile Page, and rename it under Custom Titles.

Add Smart Forms to your club page

  1. Go to Settings → The Club.

  2. Open the Appearance tab.

  3. Under Club Home or Club Profile Page, click Add modules.

  4. Choose Smart forms.

  5. Drag it to where you want it.

  6. Click Save.

Rename the section

  1. On the Appearance tab, scroll to Custom Titles.

  2. Enter a name in the Smart Forms field, such as "Forms" or "Applications".

  3. Click Save.

Leave it empty to keep the default name.

Share a direct link

  1. Click the link icon in the admin top bar.

  2. Copy the Smart forms link.

  3. Share it with your players.
