Go to Settings → The Club → Appearance and choose the details under First Checkout Fields. Players must fill these in the first time they complete a purchase at your club. They are only asked once.

Go to Settings → The Club.
Open the Appearance tab.
Scroll to First Checkout Fields.
Select the details players must give.
Click Save.

Each field is asked once, at the player's first checkout at your club.
Communications email is only asked if the player's main email is private, for example when they signed up with Apple and hid their email.
Phone number, gender, address, date of birth and communications email are included when you export your player list.
