Players find your Smart Forms on your club page in the Nettla mobile app, where you placed the Smart forms module. The image below shows an example.
Good to know
What players see depends on how you set up the form: its name, description, questions and document upload.
If the form uses email domain verification, players are asked for that email address and a code before they can submit.
Each player can submit a form once.
