In Operator Admin, go to:
1️⃣ Players
2️⃣ Smart Forms
3️⃣ Create new
Name and Description will be displayed to the players who access the Smart Form in the Nettla mobile app.
You can decide to:
Require extra information, such as Gender, City
Ask questions that the player can answer
Select if a file/image upload is mandatory with the players submission
This allows you to view any kind of proof that you need from the player before you give them a certain benefit after their submission
When players then submit the form, you can view the player in the Submissions tab (admin), and you can add them into a cohort that you have set up or give them X club wallet from a Club Wallet that exists at your club.
